How do I get a Health Card?
To apply for a card, you must:
· go in person to your nearest Ministry of Health and Long-Term Care office, and; · fill in the Registration for Ontario Health Coverage form and; · provide the necessary original documents.

What if my Health Card has gone missing or is damaged?
You should immediately call the Ministry of Health and Long-Term Care at 1-800-268-1154 (Toll-free in Ontario only). In Toronto, call 416-314-5518, TTY 1-800-387-5559.
If you had a photo Health Card, you will be sent a new one. If you had a red-and-white card, you will have to register for a photo Health Card. If you find your lost card, keep it only until your new card arrives and then destroy it because it will no longer be valid. Make sure you have your new photo Health Card before you destroy the old card.

What do I do with a deceased person's Health Card?
The health card of a deceased person must be returned to the Ministry of Health and Long-Term Care. You will need to complete a Change of Information (form 280-82) and then mail it with the health card of the deceased person to the ministry. You should include a copy of the death certificate. Copies of this form are available by:
· Contacting your local ministry office and requesting that one be forwarded to you; · Printing a copy of the form through Forms Online. Alternately, you can send a letter to your local ministry office providing the deceased person's name, date of birth, sex and health number. Enclose a photocopy of the death certificate and the actual health card.

How do I obtain a Birth Certificate, Marriage Certificate or Death Certificate?
Applications for birth certificates, marriage certificates and death certificates are available in my constituency office and online (click here). Applications can be received and submitted by fax, provided the applicant is making payment by VISA, AMEX or MasterCard.

I am moving, how do I change my address (Ministry of Health, Ministry of Transportation & Ministry of Natural Resources)?
Your Health Card, Driver's Licence and Outdoors Card should all contain your correct address. Furthermore, these three ministries require correct mailing addresses in order to contact you or renew your cards. Please click here to renew your address.

What is Telehealth Ontario's Phone Number?
By calling toll free 1-866-797-0000 anytime day or night; callers will be able to get health advice or general health information from experienced Registered Nurses. Callers will be asked to describe their symptoms and answer questions to best assess the seriousness of the problem so that a nurse can help you decide your best first step. They can help you decide whether to care for yourself, make an appointment with your doctor, go to a clinic, contact a community service or go to a hospital emergency room. The service is free and confidential; callers do not need to provide their OHIP number.

What should I do when an Energy Marketer comes to my door?
When electricity retailers come to your door with contracts, read them carefully and ask questions. You don't need to do anything immediately. Electricity will still come to your home or business the same way it always has because your current utility company is the "default option" if you don't sign with a new electricity retailer. If you do sign a contract, the government has legislated a ten-day "cooling off" period during which time you can change your mind and opt out of the deal.
Here are a few tips to keep in mind:
· Make sure you are dealing with a company that is licensed by the Ontario Energy Board. · Don't sign anything until you have an opportunity to study the contract carefully and compare it to your current charges. If you utility company hasn't included the cost of the electricity or gas you should ask for it so you can compare. · Ask about extra charges you have for rented equipment, such as a water heater or a service contract that pays for furnace repairs. If you have concerns about a marketer please contact my office. You can also call the Ontario Energy Board at 1-877-632-2727, or visit their web site at www.oeb.gov.on.ca.

How do I recommend an outstanding volunteer for Provincial Recognition? The Ontario Government recognises volunteers from all sectors through the following programmes: · Volunteer Service Awards (VSA) - in recognition of continuous years of service with a community organisation. · June Callwood Outstanding Achievement Award (JCOAA) - awarded for superlative volunteer work undertaken by individuals, groups or business. · Ontario Medal for Young Volunteers (OMYV) - presented to outstanding young volunteers between the ages of 15 and 24.
